New Member Meducation Assessment

A New Member Meducation Assessment includes several parts.  First of all, before you arrive for the visit, Dr. Hersch will have reviewed all relevant records you have sent to the office.  He will also have reviewed the additional assessment tools used by Meducation.

Once you arrive in the office, you will be given a brief orientation to the office (for example, a description of policies and procedures for appointments and insurance billing, HIPAA notifications, directions to the bathroom, etc.).  Then you and Dr. Hersch will begin an age/gender appropriate history, examination, counseling/anticipatory guidance/risk factor reduction interventions, and the ordering of appropriate laboratory/diagnostic procedures.  You will be given instructions on the use of PRN for your diagnostic results, and a follow-up appointment for a phone or office visit will be scheduled.

The follow-up appointment will discuss the diagnostic results, and provide preventative medicine counseling and/or risk factor reduction intervention(s).  You will be given a written Health Management Plan summarizing your Meducation Assessment.  The Health Management Plan will include discussions of medications (including prescriptions), recommendations for changes in lifestyle, advice regarding psychosocial issues, and possibilities for alternative healthcare options.

Your medical record (including the Health Management Plan, Oregon Power of Attorney for Healthcare and other legal documents in your record) will be encoded onto a USB thumb drive.  This USB drive can be attached to your keychain and can be carried with you.  Should an accident or hospital admission occur, your medical record is always available.

Continuing Member Meducation Assessment

This is similar to the New Member Meducation Assessment but it is completed in one visit rather than two.  In addition, it does not include a review of old records—only new information.  It is a periodic comprehensive preventive medicine reevaluation and management of an individual including an age and gender appropriate history, examination, counseling/anticipatory guidance/risk factor reduction interventions, and the ordering of appropriate laboratory/diagnostic procedures.  In this case, you can complete your laboratory/diagnostic procedures before the scheduled Assessment so that your results are available for evaluation during the appointment time.

You will receive an updated Health Management Plan, and your medical record will be updated on the USB thumb drive.

Temporary Member Meducation Assessment

A Temporary Member Meducation Assessment will be one visit which will include a brief Membership Interview and orientation with the physician.  The office visit will include an appropriate history, an examination appropriate to the presenting problem(s), and a Health Management Plan focused on the presenting problems(s).  It will also include a plan for follow-up laboratory/diagnostics and a schedule of follow-up visits (including the Temporary Membership termination visit which formally returns your care to your referring physician).
